Meet Claire · Founder & Lead Practitioner

I built ELSS for people like us.

Counsellor, case manager, foster carer for 20+ years, and a neurodivergent woman diagnosed later in life. I walk alongside participants and their carers because I’ve walked there too.

Always. One key worker, your whole team joined-up.

Who I am.

I’m Claire Burton, an experienced counsellor and case manager providing NDIS and private services to Gold Coast and Brisbane residents, with remote services available across all other regions of Australia.

I’m a neurodivergent woman diagnosed later in life, and I deeply understand the frustrations of living in a world that often feels out of sync.

Why this work.

I spent over 20 years as a foster carer for children and youth with complex needs, and I have four members of my own family on the NDIS. Being part of complex government and non-government systems has given me the real-life experience and skills to genuinely walk alongside participants and their carers.

As the owner and operator of Evolution Launch Support Services, I offer a comprehensive range of services designed to empower individuals and families to achieve their goals and live fulfilling lives.

How I work.

I am committed to providing innovative, person-centred support, and to always being honest about what I can and cannot do. If I’m not the right fit for what you need, I’ll say so — and help you find the right person.

Our Approach

The Key Worker Model — and why I use it with adults too.

Traditionally used with children, the Key Worker Model puts one trusted person at the centre — creating cohesion, reducing overwhelm, and ensuring every support is working toward the same goals. I bring it to adults, because adults deserve it too.

Without a Key Worker

Holding everything together yourself.

  • Juggling 5+ separate providers who don't talk to each other.
  • Repeating your story to every new face that joins your team.
  • Carrying the mental load of "who's doing what, when".
  • No one holding the bigger picture when life shifts.
  • With ELSS

One person who joins the dots.

  • I sit at the centre of your support team — calls, notes, follow-ups.
  • You tell your story once. I keep it safe and pass on only what's needed.
  • A therapeutic lens on every part of coordination — not task-management.
  • Relationship-based, trauma-informed, and recognising the weight you carry.
  • This is not simply task management. It is relationship-based, trauma-informed support that recognises the emotional weight carried by participants and carers every day.
